Saving data to Planning DSO (ADSO) with Comment is unpredictable

I am having a ADSO ( Direct Update and Planning Mode ) with a few dimensions like G_Account , functional area etc and a comment field and few measures. I have not put any characteristic relationships or logic so its a simple save to DSO . But the saving to DSO is unpredictable , sometimes it does not save even though I get AO popup saying that the data has been saved . I have tried different combinations of the query , but the result is same . The save always does not happen .

Note : I am using S4HANA 1610
