SAPSQL_ARRAY_INSERT_DUPREC error when activating 1099 NEC form SFPF_IDWTCERT_US_1099NEC

Following manual step of 2939225 for US legal changes 2020, when saving/activating form form

SFPF_IDWTCERT_US_1099NEC, the system is resulting in run time error

SAPSQL_ARRAY_INSERT_DUPREC at the location

cl_fp_wb_object->SAVE_OBJECTT().

On debugging, found that the table FPCONTEXIT where the exception is raised when trying to insert the form does not even have the entry for form object SFPF_IDWTCERT_US_1099NEC.

Did anyone else face similar issue. Any insight on how to resolve this is appreciated.
