Issue1: ‘Return to’ is not working as expected- Ended with Cancelled, manually triggered UAR not generated properly

Explanation - We have scheduled a UAR report through Background Scheduler (Recurring activity: 3 months). Recently it was executed on 01stJuly and the request sent to all the role owners.
In that, we have faced an issue with Request number#4984.
But the user didn’t receive the UAR linkin his work inbox & Path status is ‘Cancelled’ and the stage status as ‘Error’.
Our Question is:
As we had the same stages/path for 'Return To', why the path status is changed to 'Cancelled' & stage status as 'Error' while clicking on ‘Return To’?

Our Question is:
As these 39 roles belong to the same owner(Muthu), may we know the reason for triggering 20 reports for the manually triggered UAR?
Could you please help us to solve this problem?
As well, if possible help us to re-generate the same 4984 to the Role owner(Muthu)?
