GRC Offering an Invalid Mitigation Control - #MIT_DELET

Hi, experts.

During an access request, no matter the stage when the risk analysis takes place, system is offering an invalid (blank) mitigation control as a choice for the approver.

Any suggestions on how to understand what is happening please?

The compliance approval application is generating the empty entry. But I still don't understand why it would do so.
