GRC 10.1 Rejection email is not sent to requestor

I have configured Access Request for Firefighter access.

The notification settings are set up as per below.

If the request is approved - requestor gets notified correctly

however if request is rejected - there is no notification email and request is simply closed.

If I change Rejection level to "Request" then user receives Approval notification not the Rejection. I assume that is because the line item still has Approved?

In addition how do I set up a notification to the Security team of an approved request?
