Make fields in item additional Information PROBC field as Ready only based on Item status change

Hi PPM Experts,

I have requirement to make the questionnaire assigned to the Probability of commercial success as ready only based on status change. 

 

When I click on the questionnaire button I want all the fields to be read only when Item status changes to approved.
