Please follow the steps in Output Management – Customize Email Form for Payme... - SAP Community. 

Note: Although this article mentions Payment Advice, the email template setup for Account Statement is the same. For example, in app Output Parameter Determination, select "Account Statement" for the field "Show Rules for" instead of "Payment Advice".
