GRPO With Approved Status. But appear with Reject Status in Approval Status Report

I have GRPO With Approved Status. But when I run the Approval Status Report I find it With a Reject Status

Can anyone help me to know the reason for that 

Because I can't cancel this GRPO or Even Copy it to the AP Invoice

is there a multi level approval. I.e. how many approvers are there for this document.
