Form template name cant be initial error while output determination of Maintenance Notification

We have configured the output parameter determination (Email) for maintenance notification overview on SAP S/4HANA Public Cloud, but it is not working when creating a maintenance notification. (Form template name cant be initial) error occurs during output generation.

 Please help us for this error.

Yes, this error has been resolved, but I need assistance in setting up an automated email notification for when a maintenance request is created. Currently, we only receive an email after we manually click on output determination, which is not efficient. Could you help me implement a system where an email is automatically sent out upon the creation of a maintenance request? This would streamline our process. Thank you for your assistance!

I am not aware about maintainence, For Finance we are able to schedule jobs to automate the process.

In the app Output parameter determination, check if the form templates are assigned to maintenance notification.

Thank you for the reply. Yes, I have assigned form templates to the maintenance notification, but errors still occur.
