Please use the data Source: FDP_OM_FORM_MASTER_SRV

For detailed guide please refer to the 1LQ Scope item set-up instruction of the Solution Scenario: Output Management (1LQ) section 4.3.1.1.2.1  Downloading Default Master Form Templates and 4.3.1.1.2.3  Uploading the Modified Template.

The standard Master form template is SOMU_FORM_MASTER_A4_E.
