Social Media Integration for Lead creation via Promotion Forms on Respective platforms

We have a Promotion Page in Social media Platform (FB,Instagram etc) once we will click on that it navigate a form where we need to fill the basic details. Once we submit it store those details in Facebook database as a lead.

Is there any way to flow back those lead from Facebook to C4C ?
