With regards to Lead score - it is a score (points) you give to your lead. You can set up a survey with scoring, that includes some questions (criteria as you called them). With the activity planner you make the survey appear in your lead transaction. After the survey is filled, with a workflow you update the lead score field with the score from the survey. With a workflow you can based on the leadscore, add Qualification Level cold, warm, hot to your lead.
