Use case : The first visit is the most important, so the report must shows only the first visit for each account (which have the first start date)

How can only have the first visit only for every account to have this result : (Capture2)

Visits are technically all equal. There is standard no real standard first visit indicator. But you can set up one by defining a extension field. A visit is a first visit when the Last Visit Date/Time in the visit is empty.
Unfortunately this Last Visit Date/Time is not included in the fields that appear when you want to set up a workflow rule for visits. If it was you could use it for setting up a condition and use it as a trigger to update your 'first visit indicator'.
You could create an extension field "First Visit" yes / no, make it mandatory and let the sales make a selection in every visit and include this extension field in your reports for filtering. Alternatively you can do the same with a mandatory survey assigned to the visit and make use of the survey related data sources for setting up a report.
If you really work with a specific visit type or visit category in your visit, you can then of course use them in your report selection.
