S4 Customer Management - Dates are not getting reflected in WebUI

Hello Experts,

When I am trying to create a service request the dates are not getting populated for "Requested Start, Requested End ", as the required configuration has been maintained and also i did assign the date profile to the required SRQI Item Category. Request your valuable suggestion to resolve the issue.

Hello Experts,

I did all the required config. settings, now the show stopper is the below message which is appearing while creating the service request " Service product CSSRV_01 not found", request you to please suggest on the same.
