Not able to launch form after tenant refresh

Hi All

We have 2 tier solution. For the test tenant we had a refresh last week(Copied Namespace from Prod say N1).

After the refresh I created a patch(With Namespace N2), now custom form is available in SDK(With Namespace N2) but not available in Form Template Maintanence.(neither with N1 nor N2)

Also, in SDK on launching of Form ,Form Group code is of Namespace N2.

I am not able to launch the form now due to this issue.

Please help on this. Thanks in Advance.
