You can use Context based codelist for such requirement. Please have a look at following:

That's because you have not selected anything in Parent/first dropdown. So ideal is to select the first drop down and then system will do the filtering based on this for second drop down.
Also always try to use screen from actually UI and not as a preview because in my experience i observed that sometimes UI is not behaving correctly when previewing but when accessed from browser then correct results are produced.
