CRM service request subject and reason category 1 to 4 deleted

I accidentally deleted the category 1 to 4 fields in the subject block area. Could somebody please help me fix this problem? Many thanks.

there must be another reason. The screen that you've shown seems correct too me because SubjectCatgoriesDDLBVA is a block.

My system looks like yours system.

Honestly, I think that you cannot display the categorization because you don't have any active categorization schema for subject. Check it

Thank you for your kindly help. I think my categorization schema has been activated because I am able to use the checklist function.
