Hello all,
I was wondering if it is possible to be able to capture responses from forms embedded in a landing page in Marketing Cloud. For example, here is a form that I am working on:

Note that these 4 radio buttons are actually mapped to Permission opt-out for the communication medium 'Email', i.e., ticking any of these 4 buttons would reflect an opt-out for email marketing via Marketing Cloud. At the same time, I am looking to capture the reason(s) why someone would opt-out for email marketing permissions. Is this possible?

From the Segmentation, you can identify these contacts and export them to excel with their comments for Opt-Outs.
Cannot think of other options.

On the Form, you can add the note and mark it relevant for 'Permission' to capture the reason for Opting-Out and even make the note mandatory when the contact opt-out as shown below:

In this way, we can capture the reason from contact and tied it up with the permission.

Thank you for your suggestion. I have tested it, and it works at the granular level (contact). Do you know if it is possible to export these comments to a report etc.?

I was wondering if it is possible to NOT have free-form text for the reasons + ability to export these reasons to a report so that the user is able to analyze any opt-out trends.

With the new release 2005 it is possible to capture interaction attributes in Forms. See here.
So I think it would work if you create a new a new interaction type (Z_OPT_OUT or something like that).
You can then define a custom field for Business Context: Interaction (e.g. YY1_INTERACTION_REASON).
In the Form you then have to use an interaction (where you choose your newly created interaction type) and an input field (where you can store the reasons) which you assign to the interaction field.

Then besides the real email Opt-Out interaction your custom interaction type with the reason why the contact opted out is stored for the contact.
As you can see in the screenshot, by default there are already four fields for interactions that you can use to store your values in (Content Subject, Object ID,...).
Maybe that works for you.
As an other approach I can only imagine that you first try to send the data from the Form to your CPI (like External Landing Page Integration), use an API according to your needs (where you can use the PermissionNoteText field, I think API_INTERATION_CONTACT_SRV would work), map the fields there and store the values of your reasons in that PermissionNoteText field. But not sure if that would work..
So I think the first approach is the one you should focus on.

For the first approach, I understand that the Input field would be free-form text? Is there any way we can create fixed answers? (e.g. A) Content is not relevant B) Too many emails etc..) Ideal scenario is to be able to extract these answers to a report to analyze any trends resulting in these opt-outs.
Is such a functionality possible?

since the input field is a Custom Field, you can decide what type it should be. If you use type Codelist, you can then choose the values in the form (s. screenshot).

In the Custom Fields & Logic application you can also enable the custom field for the reporting.
I think Kameshs approach is very good because the value is directly stored on permission level. But there I think you have to provide the note field in the Form always as free text and not as a Dropdown option with fixed values.
You have to check which approach better fits to your needs.
