Assume that I have added an append structure to a standard SAP table. what happens to the standard table when a new version of the table is imported during an upgrade?

Hello,

Assuming that SAP has not made any changes to the table that you added an append to, the standard table will remain the same. The append will still be there as well.

If SAP delivers changes to the table it will show up in SPDD and you will be given the chance to reinclude your append. If for example you created the append because of an SAP note it is possible that SAP will incorporate the fields that you added in the append into the new version of the table delivered with the upgrade. In that case you will want to discard the append.
