SAP GRC 10.1 HR Triggers Not Terminiating

I have tried to configure HR Trigger based termination on the GRC system. Here are the steps completed.

1. In HR System SPRO Plugin Configuration settings.

2. In GRC

1. Copy ID from BRF+Function Mappint.

2. using the ID, create the decision table and required rules in BRF+ transaction.

3. Test simulation and get good results. Everything according to the Wiki article for HR trigger configuration.

All i want to do at this point is lock account and put term date. Now when I try terminate the user in PA40, it does not lock/term account.

This is what I see in SLG1 in the GRC system.

Not sure where the problem is.

Can anyone suggest on how to proceed, or on how to troubleshoot this.
