Error while displaying HTML template

Hi all,

I have downloaded the existing HTML template and made some modifications to it.(I have just added four radio buttons)but now I am getting error that "Error has occurred in the script of the page".

I tried uploading the old version of the template but still this error is coming.

Any pointers for this are welcomed.

which template you have modified.

can you post the modified code here so that we can check whats wrong?
